Rochester, NY… We are pleased to announce that Catholic Family Center is now accepting referrals to five Home and Community Based Services (HCBS) programs, starting September 15th. These services are exclusively available to HARP eligible members who are referred by Health Home Care Managers. We are ready to begin providing the services, but the first step of the process for your members starts with you!
These programs are designed to expand access to person-centered, trauma-informed support and training services provided by our compassionate and capable clinical staff and peers in settings that best suit your client's needs. These services are recovery-oriented, supporting care managers' integrated plans of care addressing physical and behavioral health needs through evidence-based and data-driven techniques that can build stability and promote hope, encouraging each person on their individual path to recovery.
The referral process for HCBS is simple, and thanks to the assessment and eligibility criteria built into the HCBS model, it's easy to know which of your Managed Care clients are eligible. Once you've completed the HCBS Health Home Assessment, you can refer your clients to any of our HCBS programs through any of the following channels:
